For this assignment, you will demonstrate your knowledge of case management practice by reviewing the case of Maria and then completing the following tasks:
Provide a social history of Maria that includes the following:
A summary of her presenting problem
Summary of her background information
Your impressions and recommendations for case management
A treatment plan that includes a minimum of 2 goals with two corresponding objectives.
For each goal you identify, provide a narrative of the interventions you would use to assist her in meeting the goal.
Create a list of referrals (either locally or nationally) that would be appropriate for her.
Your resource list is required to have a minimum of 3 referrals,
Remember to be specific and provide the name, address and contact information for your resource and a brief narrative (minimum of 5 sentences) as to why she would be appropriate for this resource.
Create a contact/case note of the transcribed session utilizing either the SOAP or DAP format (see module 4 for assistance)
Remember to include the date and time of session
Remember to include a follow up plan.
Ensure that the pages are exclusive of the cover and the reference pages. Also, ensure that you include all the references you use in finding research for this assignment paper. References should be at least three for the paper. All references, citation, and writing should follow the APA formatting and styling guidelines.
